We are currently seeking an experienced hourly Nurse Case Manager
Key Responsibilities
Case Management, guided by the company?s service model, which encompasses professional resources management, goal setting, team management, case budgeting and third party funding approvals
Setting rehabilitation goals in consultation with clients, their families, case teams and other treating health professionals
Facilitating medical evaluations and home modifications
Key Qualifications
Demonstrated knowledge of best rehabilitation practices and case management principles
Clinical experience with spinal cord injury and traumatic brain injury considered an asset
Superior interpersonal relations, communication, problem-solving, and time management skills
Demonstrated ability to work collaboratively as a member of a multidisciplinary team
Experience adjudicating and managing sick leave plans for employer/employee organizations considered an asset
Self -motivated with demonstrated ability to manage a busy and varied caseload
Completion of a satisfactory Police Check
