Job Title: DHA/DOH Licensed Nurse for Elderly Care

We are seeking a skilled DHA Nurse with expertise in elderly care and a passion for training and developing a caregiving team. Key Responsibilities:
- Elderly Care: Provide compassionate in-home nursing care to elderly clients, including medication administration, monitoring vital signs, and managing chronic health conditions. Assist with daily activities, personal hygiene, mobility support, and meal preparation. Develop and update individualized care plans with input from clients, families, and healthcare professionals, responding promptly to changes in health status.
- Patient and Family Education: Provide essential health education to patients and families, focusing on effective condition management and medical equipment use.
- Coordination and Communication: Work closely with physicians and other healthcare professionals to ensure continuity of care and accurate patient status updates.
- Documentation and Compliance: Maintain precise and up-to-date patient records, adhering to safety protocols and infection control procedures, ensuring that all care practices align with DHA/DOH regulations.
- Training and Development: Design and implement training programs for new and existing caregiving staff, covering best practices, DHA compliance, and safety protocols. Lead workshops, hands-on demonstrations, and evaluate training effectiveness, providing ongoing support and mentorship to nurture a culture of excellence.
- Documentation and Compliance: Maintain accurate client records, including care plans and incident documentation. Ensure compliance with DHA standards and homecare policies, participating in regular audits and reviews to uphold quality and safety standards.
Qualifications:
- Education and Certification: Bachelor's degree in Nursing or equivalent, valid DHA/DOH license, and geriatric nursing certification preferred.
- Experience: Minimum 3 years in elderly care, ideally in a homecare setting; experience in training and team development is highly desirable.
- Skills and Competencies: Strong clinical expertise in elderly care, effective communication, mentoring skills, ability to adapt in a dynamic home care environment, and proficiency in EMR systems.
- Outstanding interpersonal and communication skills, with a compassionate approach to patient care.
- Flexibility to work independently and adjust to the dynamic nature of homecare environments.
